Ø Receiving medical care and treatment without discrimination based on race, faith, color, national origin, language, ancestry, religion, gender, sexual orientation, marital status, age, disability, health insurance, economic and social status.
Ø Receiving a satisfactory level of service in terms of medical ethics, international quality standards, technology, comfort and human relations.
Ø Knowing that priority in services is given objectively, based on law and medical criteria.
Ø Knowing the names, identities and professional titles of the people I serve.
Ø Asking for information in a way that I can understand about my health status, the medical procedures to be applied, the benefits and drawbacks of the procedures, the course of my disease and alternative treatment options.
Ø Knowing the relationship between other health or education institutions regarding my service process.
Ø Examining my medical records to the extent permitted by law and requesting corrections when necessary.
Ø Receiving information with a gentle expression that is respectful of my values, beliefs, and philosophy of life and appropriate for my spiritual state.
Ø Knowing that I am at a University Hospital and some healthcare providers are being trained.
Ø Choosing and changing the physician, healthcare personnel, health institution and treatment method from which I will receive service after being informed.
Ø Requesting that my medical documents, source of payment, and all contact information be kept confidential, even after death, except where permitted by law.
Ø Knowing that there will be no medical intervention for research or educational purposes without the approval of the Ministry of Health, me or my legal representative, and that my name will be disclosed when I participate in the research, again with my consent.
Ø Givingapproval before starting all kinds of diagnostic methods and treatment, except in emergencies and according to the procedures specified in the law.
Ø Knowing that when I am unable to express my wishes, my requests such as being an organ and tissue transplant donor will be fulfilled in accordance with laws and regulations.
Ø Reporting my complaints, thanks or suggestions, reviewing my complaint, concluding it and informing about the result, without prejudice to my legal rights.
Ø Receiving compensation if I suffer physical, moral or psychological harm during my treatment.
Responsibilities of Me and My Legal Representative as a Patient
Ø Mevcut şikâyetlerim, geçirdiğim hastalıklar, uygulanan tedaviler, kullandığım ilaçlar ve sağlığım ile ilgili diğer tüm bilgileri zamanında ve doğru olarak bildirme.
Ø Tedavi süreci boyunca beklenmeyen değişiklikleri, oluşabilecek uyumsuzlukları hekimim ve diğer sağlık çalışanlarına bildirme.
Ø Bakım ve tedavime en geniş ölçüde katılma, her konuda tüm çalışanlarla işbirliği içinde olma.
Ø Yapılacak işlemler, tedavi, ilaçlar ile ilgili tavsiyeler ve benden beklenenleri anlamadığımda mutlaka sorma, uygulamam söz konusu olamayacaksa hekimim ve diğer sağlık çalışanına bu durumu bildirme.
